2. Sports Officials Information Sheet - PDF - or - Word Format
3. Availability Calendar  - PDF - or - Word Format
4. Check for SDFVOA dues ($50.00)  - make check payable to SDFVOA
5. Check for Assignment Fees ($75.00) - make check payable to Debby Ahrenstein
6. Proof of Insurance (list of vendors will be handed out at the first meeting)
