Fill out the form below to submit your uniform designs to our organization.  Please provide us with a photo of the front and back of each uniform so we can compare them to each other and make an informed decision. 

We will review the design and confirm whether it complies with NFHS regulations and notify you via email once we have made our decision.

Reminder: this process is to help avoid your team being penalized at the start of a match or losing access to a libero due to non-compliant uniforms.
